Community Health Worker Program (CHWP) is a case management program that provides health and family support services to 200 pregnant and parenting families in East Harlem. CHWP services include advocacy, referrals, home visits, and outreach and child health insurance enrollment services. Community Health Workers are recruited from the East Harlem community, trained and sent back into the community to locate, enroll and service the needs of mothers and babies. This program serves those who are homeless as well as those residing in zip codes 10029 and 10035. The Community Health Worker Program is funded by the New York State Department of Health.
